Care Manager – Dundrum or Dun Laoghaire

Established in 2014, Heritage Homecare has grown quickly to become established as one of the leading providers of homecare in South Dublin. We have two office locations: Dun Laoghaire and Dundrum and as well as serving private clients, we are an Approved Service Provider for HSE areas CHO 6 (Dublin South East and Dun Laoghaire) and CHO 7 (Dublin West and South West).

Arising from continued expansion, Heritage Homecare is looking to recruit an additional full-time Care Manager to join its growing team. The Care Manager will report to the Director of Care Management and could be based either in Dun Laoghaire or Dundrum. This is a great opportunity to join a growing company at an exciting stage in its development. The successful candidate would be joining a friendly team who work hard in a supportive environment.

The main functions of the role are:

Assessing the homecare needs of new Clients and communicating the benefits of our service in a professional and reassuring manner
Dealing with fellow healthcare professionals in hospital and community settings
Creating and documenting Client Care plans
Initiating the service to Client’s by matching Carers with the preferred expertise and profile
Conducting Client Risk Assessments and overseeing the Quality of the service
Interviewing and recruiting potential Carers
Providing ongoing support for ‘existing clients’ managing medical and HSE-related matters
Participation in on-call rota (one week in five approximately)

The successful candidates may be a qualified nurse or have a different healthcare qualification relevant to the role. In addition, the ideal candidate should possess most of the following:

Experience or a strong interest in working with the elderly in a community setting.
The ability to multitask, to work under pressure and to use their own initiative.
Capable of prioritising and problem solving and they should have excellent attention to detail.
Be confident communicators and have the ability to influence and reassure people of all backgrounds.
An energetic and positive demeanour and an ability to judge situations and people and to think on their feet.

Knowledge of the health and social challenges facing the elderly, and previous experience in the homecare sector would be a distinct advantage.
HR / Recruitment experience would be a bonus but not essential.

The office hours are 9:00 - 5:00pm Monday to Friday. Salary is €40,000 - €45,000 negotiable depending on experience. + annual bonus related to the performance of the business + Company Car or mileage expenses.
